Hi Scouts and Parents,

Need Medical Forms Part A, B1 and B2 filled out and a copy of your medical insurance card front- back is due by March 16th. 
Need Part C  is due by May 18th for Summer Camp. 

Please read all of this email. It’s that time of the year that we need your medical forms filled out.
Only use these Medical forms that are attached, or we have copies at the Scout House. 
Please answer all questions.
Part A - Update information and date this year.  
Part B1 - Update information and add Unit Leader: Vern Green, Unit leader's mobile # (805) 794-7255. Council: Ventura County, Unit No.: Troop 128.
Part B2 - Update information and fill in the dates for all vaccines, including Covid-19. For Adults your tetanus shot needs to be current, it expires every 10 years.
Need a copy of your medical insurance card - front and back. If you have not changed medical insurance from last year, we should have a copy already on file. 
Part C - Doctor fills out. Expires 1 year from the date the doctor signed.
 
Please turn Medical Forms in the basket on the table next to the restroom in the Scout House.
