Hello Troop 128!

It’s that time of the year where we need updated medical form.

Scouting adventure, camping trips, high-adventure excursions, and having fun are important to everyone in Scouting—and so are your safety and well-being. 


Completing the Annual Health and Medical Record is the first step in making sure you have a great Scouting experience. So what do you need?


All Scouts and Registered Adults must update their Health and Medical Forms annually for the troop to keep on file in order to participate in activities.


Complete the following as soon as possible.

FORMS PART A AND PART B1/B2 ARE DUE BY FEB 29!

  1. Medical Form Part A - Complete with updated information and date this year.

    1. Make sure to check the “None” box if there are no participant restrictions

    2. Both the Scout and a parent/guardian need to sign and date this form

  1. Medical Form Part B1 - Complete with updated information and add:

    1. Unit Leader: Vern Green

    2. Unit leader's mobile # (805) 794-7255

    3. Council: Ventura County

    4. Unit No.: Troop 128

    5. A parent/guardian needs to answer “Yes” or “No” to the non-prescription medication question and sign under this section.

  1. Medical Form Part B2 - Complete with updated information

    1. For Scouts, make sure to fill in the dates for all vaccines, including Covid 19

    2. Adults and Scouts must have a current tetanus shot. It expires every 10 years.

  1. A copy of your medical insurance card - front and back.

    1. If you have not changed medical insurance from last year, we should have a copy already on file.

Medical Form Part C is required for High Adventures, Kayak Trip, and Summer Camp participation.

FORM PART C IS DUE BY MAY 30!

  1. Medical Form Part C - To be completed by a doctor

    1. This form expires one year from the date the doctor signed it.

Here's the link to download the fillable PDF file that you can complete and print at home. I will also have printed medical packets available in the Scout House for those that need them.


Please answer all questions and return completed and signed Medical Forms to the basket on the table next to the restroom in the Scout House or to me directly.
